How To Use A Quiz As An Intake Form

Step 1: Install and Activate HD Quiz

The first step, of course, is to install and activate the HD Quiz plugin on your WordPress website. You can do this by navigating to your WordPress dashboard, clicking on “Plugins,” and then selecting “Add New.” Search for “HD Quiz” and install it. Once installed, activate the plugin.

Step 2: Create a New Quiz

After activation, you’ll see a new option in your WordPress dashboard menu called “HD Quiz.” Select to begin the journey of creating your very first quiz. HD Quiz was designed and developed from the ground up to be as easy and intuitive to use as possible, but I also have extensive documentation as well as a tutorial video.

Step 3: Purchase Save Results Pro

The Save Results Pro addon was developed by me (Harmonic Design / HDPlugins) the developer of HD Quiz, and it will allow you to add custom fields to either before the quiz (all fields need to be filled out before the user can take the quiz), or before the results (all fields need to be filled out before the user can submit their quiz and see their results).

At minimum, you can use this to collect the name and email address of your customers, but you can also add as many custom fields as you need to do things like ask who your student’s teacher is, or to ask how a customer was referred to you, etc.

This addon will also capture each individual question and selected answer, which makes this a very powerful tool to help categorize your user’s responses.

Step 4: Test and Preview

Always test to ensure that everything works smoothly. Preview the quiz from a user’s perspective to catch any errors or inconsistencies.

Step 5: Publish and Promote

Once you’re satisfied with your customer intake quiz, publish it on your website. Promote it through your marketing channels, social media, and email newsletters to maximize its reach.
