Form after Quiz not Loading

I have an issue with the form I made not being shown after the quiz has been taken. When I do the simple form at the beginning it works.

I just took a look and can see the form after your quiz as expected.
Are you using a page caching plugin? If so, you’ll need to clear your cache after changing settings.
