Cannot add HD Form – Editor loads incorrectly
Hi Dylan,
I added Save Results Pro and HD Forms to my site with the intention of building a form in HD Forms and adding a custom field to the form before my quiz starts. However, the formatting of the “Add Form” page in the back-end seems to be loading incorrectly and I’m also unable to save it and actually use the form. I’m attaching a screenshot of what it looks like for me when I click “HD Forms” > “Add New Item”

this is a new one. From the screenshot, all I can say is that for some reason your site is not loading the styles for the page.
The most likely reason is that something on your site is causing an error before HDForms is able to load.
Things you can try:
– When creating a new form, open your browser’s console and take note of any errors. You can do this on most browsers by pressing F12 and making sure the console tab is selected. Anything in red is especially important.
– If the above does not point us towards a solution, then the next step is to do the default WordPress debug steps. In your case, I’d recommend temporarily disabling all plugins except HDForms to test, and if it works, re-enable one by one to find the one causing any issues.
– Lastly, make sure to clear all caches to ensure this is not a caching issue. Clear your browser cache, and site cache if you have a cache plugin installed.

Unfortunately I’ve tried all three of these potential solutions with no luck. I also whitelisted it in my firewall settings because I thought that may be the issue, but it hasn’t helped.

Turns out it was a firewall issue with GoDaddy blocking it. Thought I’d let you know in case it comes up for anyone else. Thanks for all of your help.
