Have question. My form is for 15 questions, there is no right or wrong answers. What should I do?

If you do not have right or wrong answers, then you are not looking for a quiz maker, but maybe a contact form? I’d recommend Contact Form 7 or Gravity Forms. Both plugins will be able to do what I think you are looking for
Thank you very much for quick response! Yes, I tried it with contact form 7, however, I couldn’t find the way to make 15 questions. If I got one radio button, it’s one question. If I tried to make second question, one I made popup basically.. I appreciate if you have any idea on that also…
Yes, you just need to make sure that that radio button has a unique name.
Example:
[radio radio-hd1 use_label_element default:1 "option 1" "option 2" "option 3" "option 4"][radio radio-hd2 use_label_element default:1 "option 1" "option 2" "option 3" "option 4"][radio radio-hd3 use_label_element default:1 "option 1" "option 2" "option 3" "option 4"]
