there is the full documentation that can be found on the same page you posted this support request from, as well as links to this support forum and documentation on all the main HD Quiz pages within WordPress. Instructions on how to add the quizz to a page is also directly on each individual quiz!
There is also the full demo/tutorial video that can be found on the official HD Quiz WordPress page, and the support forum here. That video goes over how to create a quiz and also how to add the quiz to any post or page you need.
If I were to guess, you just need to copy/paste the quiz shortcode to wherever you want it to display. The highlighted area in red is what you need.
