Hi,
I wanted to share my feedback on the HD Quiz plugin that I recently tested. Overall, I must say it’s a great plugin! However, I encountered a problem with the CSV Uploader feature.
When I tried to use the CSV Uploader with the example file provided by you, I received an error message stating that the uploaded file was not a CSV.
Additionally, I attempted to import my own file, which I exported from Excel. Although the import process appeared to complete successfully, I couldn’t locate the imported questions.
I thought it was important to bring these issues to your attention so that they can be addressed and resolved. If there are any troubleshooting steps I can take or any additional information I can provide to assist in resolving these problems, please let me know. I look forward to your response and appreciate your attention to this matter.

please take a look at this previously resolved support thread from another user with the same issue:

There is a good chance that this is the same issue for you. The TLDR: is that your server is either missing some PHP extensions to help determine file mimetypes/extensions, or your host has used up all of its tmp folder (the place your server temporarily uploads files to before processing). Further down in that thread you will find a more detailed explanation as well as a possible solution.
