Hi Paul,
there is no integration with zapier or engagebay, but HD Quiz is very extendable, so you could certainly hire a developer to add this in for you.
You can include a form from your favourite contact form plugin in the results text (the pass or fail result text at the end of quizzes), but if you want to save or email the results, you will need the Save Results Pro addon (found on the addons page under HD Quiz). With the SRP addon, not only will results of the quizzes be saved, but you can also add custom forms to the quizzes to get additional information from your quiz takers.
